FAST SCALE: 40 MEDICAL HOMES IN 10 MONTHS
Description
A $4B mid-size health care system in the Minneapolis area spent 18 months transforming two traditional primary care clinics to ACO (Accountable Care Organizations)- based medical homes. Results showed lower cost, improved clinical outcomes, and a significantly better patient experience. The challenge was then how to transform the other 40 clinics. The CEO and Chief Medical Officer adopted an innovative way to scale fast and obtain the same (and in some cases better results than the first two clinics) in half the time, across 20 times the number of clinics.
Methodology
The project started with care model innovation carefully crafted within two clinics as lead prototypes. An adaptive design methodology was used. Forty clinics then went through a design process using an accelerated version of the adaptive work system approach. A Work Innovation Network model, based on high involvement, knowledge sharing and dashboard management for rapid diffusion was then employed.
Results
This project resulted in 42 clinics certified by the state as medical homes, 40 of them within 10 months. Competitors achieved, on average, less than 6 in the same amount of time.
